    REGISTRATION PAGE

This form is required for ALL players, coaches, and managers rostering with a Juventus STL team

To begin registration, please click on the REGISTRATION tab above.

 COACHES / MANAGERS:

• You should use the LOGIN you created/used when you submitted your background check. Please remember, a background check is required to be submitted each year after 7/1 for the upcoming year.
• This registration is needed EACH YEAR in order to add you in to the Juventus STL database for the upcoming soccer year in order for you to be added to a Juventus STL roster. 
• You will not be able to be added to a roster until the registration is completed.  There is no fee for this registration. 

PLAYERS:

• This registration must be completed EACH YEAR before the player can be added to the roster.
• If you already have an Affinity account, you will only need to update any outdated information.  Your birth certificate, picture, insurance, and contact information will already be in your account.  Please review the information and update it to reflect any changes to your address, contact info, insurance, etc.  The head shot picture should be updated each year.
• If you are CREATING an Affinity account:
  1. You will need your child's medical / insurance information to complete the registration.
  2. You will need to UPLOAD a clear and legible copy of your child's BIRTH CERTIFICATE
     • you will not need to do this to complete the registration initially, but WILL need to have it uploaded before they can be added to a roster.
     • if you are unable to load the BC, you can:
       • text/email me a copy of the BC to admin@juventusacademystl.com or 314-570-7857 and I will upload it
       • bring me a copy of the BC and I will upload it.
  3. You will need to UPLOAD a HEAD SHOT picture of your player
     • you will not need to do this to complete the registration initially, but WILL need to have it uploaded before they can be added to a roster.
     • the system does have a cropping function that will allow you to cut out dead space.
     • The Head Shot should be similar to a driver's license ... no sunglasses, hats, multiple people in the pic, inappropriate clothing, full body, too small, etc.
     • if you are unable to load the pic, you can text/email me the pic to admin@juventusacademystl.com or 314-570-7857 and I will upload it.
• NOTE:  Juventus Academy Saint Louis teams will select the NO ASSIGNMENT CODE button when prompted for a team assignment code.
